GOALS

Project goals focus on patient health to:

• Reduce the number of patient recidivism to zero due to issues related to high glycemic levels.
• Educate both medical staff and patients on the need for active blood sugar management at home after hospitalization.
• Teach nursing staff the proper tools, procedures, and processes for enhancing glycemic management in-hospital and at home.

Solution

Hospital management will partner with MCM to create a training and performance program including awareness, software training, patient education, and assessment components. MCM will interview hospital leaders to determine high-level goals and objectives, and interview subject matter experts to gather detailed content. The MCM team will design, develop, and implement the solution. 100% of the nursing staff will be included in the training and high level support will be built in to show the importance of the project and optimize the opportunity for global adoption.

RESULTS

A comprehensive blended solution is created to help achieve the client’s goals. MCM writes scripts, shoots video, and authors e-learning modules to create a high quality and effective training and performance program. Results show that [hard data here]. Since nurses are the front line of patient care, their increased awareness on the importance of educating patients on glycemic control contributes to improvement in patient post-hospitalization health. Nurses master software that allows the best in-hospital glycemic control procedures to be administered and managed. Patients learn the vital importance of home blood sugar management to their daily and long-term health. Unexpected results are that a multi-level stakeholder approach creates a positive collaborative atmosphere between administrators and nursing staff.
